Please return completed Midlakes hold harmless form to the Head Coach, Jada Yamada, on the first day of practice. 

Practice times are by age group and are as follows: 11-12s will practice 8-9am, 10 and unders will practice 9-10am and 13 and over will practice 10-11am.
